1. Name and date of the disaster? Iroquois Theatre fire happened on December 30, 1903
2. Where did this disaster occur? The Iroquois Theatre was at 24–28 West Randolph Street, between State Street and Dearborn Street
3. How many people died? 602 people died as a result of the fire
4. What contributed to the great loss of life? Only one entrance/exit- design flaw; lack of fire readiness
5. What building now occupies the site? The Oriental Theater Building

1. Where did this disaster occur? Northeast corner of the Loop at Wabash Avenue and Lake Street
2. Date? Chicago Loop derailment occurred on Friday February 4, 1977
3. How many people lost their lives? 11 persons
4. What was the cause of the accident? “L” motorman ignored/overrode the signals and proceeded ahead to bump forward cars that derailed, with some falling to the street

1. Name and date of this disaster? The Eastland disaster occurred on Saturday, July 24, 1915
2. Location? South bank of Chicago River between Clark and LaSalle, where River Theater is located on the River Walk. [Note: I did a River Cruise on the hundredth anniversary and was going by when the memorial wreaths were placed in the water during a ceremonial gathering that day. Touching.]
3. What was the cause? Lack of ballast to stabilize the ship. The ship rolled over into the river at the wharf’s edge when most passengers went to one side. The S.S Eastland was called the “Speed Queen of the Great Lakes.”
4. What was the occasion? Chartered for Western Electric’s fifth annual employee picnic
5. How many people perished? How many whole families? 844 people lost their lives, including 22 entire families
